Below is a link to a form called the state contract questionaire that is required in order for all early learning centers to be able to buy at the state bid prices.

For question B, the answer is Act 498 of the 2014 legislative session.

For question C, list the source and percentage of your childcare centers funding derived from public sources.

For instance if you take CCAP or the food program, put the percentage of revenue derived from publicly funded programs.

Then MAIL the forms the address at the bottom of the form

P.O. box 94095, Baton rouge LA

Once the office of state procurement has approved the form for your participation they will add your name to the database.
