Certificates celebrating the ANZAC
Between 2014 and 2018, Australia will commemorate the ANZAC Centenary. Queenslanders will have the opportunity to reflect on the courage of the men and women who served for Australia and New Zealand during the First World War.
We are one of only a few government agencies in the world that have records for First World War deaths. This gives us a unique opportunity to contribute to the ANZAC centenary by offering limited edition commemorative death and birth certificates.
Commemorative ANZAC death certificates
Families can buy an ANZAC commemorative death certificate to commemorate a fallen family member who served in the Australian Defence Force.
An ANZAC commemorative death certificate costs $28.00 (standard postage included).

How to apply
To apply for an ANZAC historical commemorative death certificate (deaths that occurred more than 30 years ago), search historical deaths and apply online or complete a death certificate application form.
